Abstract

The war has significantly complicated access to modern therapy, reduced adherence to treatment regimens, and increased the influence of socio-economic barriers. Under conditions of multi-component pharmacotherapy (up to 10 drugs according to ESC, AHA, and Ministry of Health of Ukraine recommendations), these challenges lead to decreased adherence to treatment. Objective. To assess the impact of socio-economic factors, lifestyle, and drug-related characteristics on adherence to therapy for coronary heart disease (CHD) with comorbid conditions. Materials and methods. The study was based on the results of a survey of 101 patients with CHD, hypertension, type 2 diabetes mellitus (T2DM), and chronic kidney disease (CKD) conducted between June and September 2024. Data were collected using Google Forms. The questionnaire included sections on: socio-economic factors (income, drug costs, participation in the “Affordable Medicines” program); behavioral aspects (attitudes toward treatment, responses to improvement/worsening of health status); preferences in the choice of medicines (innovative vs. generic drugs, trust in physician/pharmacist); and assessment of adherence to pharmacotherapy of CHD with comorbid conditions (Morisky-4 scale). The analysis compared patients who consulted only physicians with those who consulted both physicians and pharmacists. Statistical methods included χ² test, φ-coefficient, and odds ratios (ORs). Results. Adherence to treatment in wartime Ukraine was influenced by a combination of socio-economic barriers, patients’ personal preferences, and the level of support from the medical community. The involvement of pharmacists in multidisciplinary care significantly improved adherence rates. Conclusions. Expanding the reimbursement program to include both innovative and generic medicines recommended by clinical guidelines, along with providing high-quality patient counseling, should be prioritized in national cardiology policy to improve outcomes for patients with CHD and comorbidities.
