Abstract
The article presents modern requirements for the provision of medical care in the context of a sociocultural approach, in which limitations are considered as the result of discrimination by society, and not the human condition. The concept of "universal design" is gaining particular importance in all areas of human life. The deaf community is considered as a linguistic and cultural minority with its own language (sign language) and its own culture (deaf culture), therefore it is very important to offer solutions and approaches to establish quality communication between sign language patients and doctors, junior medical staff and thus ensure an inclusive approach in the field of health care. Considering the relevance of the problem, the purpose of the article is to analyze the theoretical and clinical aspects of the problem of accessibility of medical (dental) services for patients with a severe degree of hearing impairment (sign language) and to determine effective methods of meeting their special communicative needs. The methods of theoretical analysis of literary sources and results of modern research, presented in electronic scientific publications, included in the scientometric databases PubMed, Index Copernicus, Ulrich's Periodicals, Google Scholar, Web of Science, Scopus, Medline, etc., were used; examination, treatment and counseling of a sign-speaking patient within the scope of surgical preparation for orthopedic prosthetics at the Department of Surgical Dentistry and Maxillofacial Surgery of the National Medical University named after O.O. Bogomolets The results of the study give grounds for drawing the following conclusions: analysis of professional literature, clinical cases testify to the relevance of the problem of communication between deaf (sign language) patients and dentists, determination of guidelines for reforming the system of medical services in accordance with modern sociocultural approaches; the main methods of communication between a dentist and a patient with severe hearing impairment are characterized: through a sign language interpreter; using written speech; by using a memo-algorithm with gestures necessary during standard dental manipulations; it was determined that the use of the memo-algorithm contributes to effective communication with sign-speaking patients, taking into account the specifics of the dentist's activity and the special communicative needs of such patients.
