Abstract

In the context of martial law, ensuring effective, affordable, and rational pharmacotherapy remains particularly relevant. To determine the frequency of prescribing drugs for the pharmacotherapy of coronary heart disease (CHD) with comorbid conditions (type 2 diabetes mellitus [T2DM], chronic kidney disease [CKD]) in order to identify trends in drug consumption, reveal gaps in drug provision, and improve patients’ access to modern pharmacotherapy. Materials and methods. A comprehensive methodological approach was applied, based on the results of an online survey of 788 physicians from 22 regions of Ukraine (June–September 2024) using Google Forms. The survey participants included cardiologists (29%), endocrinologists (18%), nephrologists (10%), and general practitioners–family physicians (37%). Statistical methods of generalization and comparison, a clinical and epidemiological approach, ABC analysis (classification of drugs by prescription frequency), frequency analysis, VEN analysis (assessment of clinical significance), ATC/DDD analysis (WHO classification for dosage and use levels), as well as content analysis (compliance of prescriptions with clinical guidelines: ESC, AHA, ADA, KDIGO) were applied. Insufficient inclusion of key drugs for the treatment of CHD with concomitant T2DM and CKD in the state reimbursement program “Affordable Medicines” highlights the need to revise and expand the drug list. It is recommended to include atorvastatin, POACs, and 2-indole glucocorticosteroids in the program, in accordance with clinical guidelines and health technology assessment. Expanding the reimbursement list will contribute to improving treatment effectiveness and ensuring better access to essential pharmacotherapy for patients during the war and post-war periods.
