Abstract
Insufficient adherence to prescribed pharmacotherapy for ischemic heart disease (СHD) in patients with comorbidities leads to a significant number of deaths in developed countries each year. The aim of the study was to determine how gender, the number of prescribed medications (polypharmacy), psychoemotional state, and quality of life affect adherence to pharmacotherapy in patients with IHD and comorbidities, in order to improve approaches to identifying high-risk groups. Medical records of patients with coronary artery disease and concomitant hypertension, dyslipidemia, atrial fibrillation (AF), and heart failure (HF), as well as cases of acute myocardial infarction (MI) with or without ST-segment elevation, planned or emergency coronary artery bypass grafting (CABG) or mammary coronary artery bypass grafting (MCBG), and percutaneous transluminal coronary angioplasty (PTCA), with or without concomitant type 2 diabetes and chronic kidney disease (CKD), who underwent inpatient and outpatient treatment (n=224), were analyzed. Prescriptions of hypoglycemic, hypolipidemic, and antihypertensive drugs were evaluated in accordance with international and national guidelines (ESC/AHA/ADA and the protocol of the Ministry of Health of Ukraine). Statistical methods included the χ² test, Mann–Whitney U test, logistic regression, and Kendall’s Tau. Female gender was identified as a predictor of reduced adherence to pharmacotherapy. Polypharmacy in patients with IHD is common, but not always justified, particularly in the presence of comorbidities. The study highlighted the need to simplify treatment regimens and to use fixed-dose combinations to improve adherence. To predict the risk of insufficient adherence to pharmacotherapy, the following regression model can be applied: logit (P) = −1.629 − 0.794 × Gender + 0.071 × Drugs_1st_visit + 0.397 × Drugs_2nd_visit.
