Pathogenetic synergy between psycho-emotional disorders and non-valvular atrial fibrillation under conditions of chronic stress in Ukraine

Abstract

In Ukraine, the profound psycho-emotional strain caused by the war has coincided with a high prevalence of non-valvular atrial fibrillation (NVAF). Under these conditions, approaches to medical care must be re-evaluated as both clinical and social issues. This study analyzes the epidemiological, pathophysiological, and clinical factors linking anxiety-depressive disorders and NVAF amidst chronic wartime stress, while considering the optimization of pharmacotherapy and the integration of mental health care into cardiology practice through individualized approaches. An analysis of international and national evidence-based data was conducted, focusing on mechanisms of neurohumoral activation, inflammation, pharmacokinetic drug interactions, and behavioral risk factors for NVAF. It was established that chronic stress—affecting up to 83% of the population—contributes to the development of anxiety, depression, and emotional exhaustion, which in turn trigger the mechanisms leading to NVAF. Psycho-emotional disturbances were found to promote atrial remodeling and cardiac rhythm abnormalities through the activation of the autonomic nervous system, the hypothalamic-pituitary-adrenal axis, inflammatory pathways, and oxidative stress. Conversely, NVAF exacerbates psychological distress through disease progression, reduced cerebral perfusion, and social isolation. It has been established that the use of complex treatment regimens for non-valvular atrial fibrillation (NVAF) carries the risk of clinically significant drug-drug interactions. Amidst the chronic stress of the war in Ukraine, NVAF should be viewed as a multifaceted condition shaped by the interplay between the cardiovascular system and mental health; this necessitates a shift toward individualized, multidisciplinary care strategies that integrate mental health screening and non-pharmacological interventions.
