Abstract

The rapid rise in the prevalence of Cardiovascular-Kidney-Metabolic syndrome (CKMs) in Ukraine is accompanied by significant polypharmacy and places a heavy burden on healthcare resources. A systematic analysis of international experience has revealed gaps in the existing system of care for CKMs patients and established a rationale for strategic guidelines to integrate pharmaceutical care into clinical practice in Ukraine. A narrative review was conducted using an evidence-synthesis approach to identify relevant publications in international databases such as PubMed/MEDLINE, ScienceDirect (Scopus), and the Cochrane Library. It has been demonstrated that the active involvement of clinical pharmacists in the treatment process leads to a statistically and clinically significant reduction in systolic blood pressure (by an average of 7 mmHg) and achieves high levels of patient adherence to prescribed pharmacotherapy (up to 90%). The study highlights the pivotal role of pharmacists and clinical pharmacists in implementing deprescribing strategies for potentially inappropriate and hazardous medications (specifically opioids, benzodiazepines, and sulfonylureas) while simultaneously justifying the initiation of modern organ-protective pharmacotherapy (SGLT2 inhibitors and GLP-1 receptor agonists). The results of the pharmacoeconomic modeling projected for 2026 demonstrated that the annual cost of preventive pharmaceutical care is 17 times lower than the cost of a single hospitalization for macro- and microvascular complications of type 2 diabetes: 680–700 USD versus 12,100 USD, respectively. Successful implementation of this model within Ukraine’s healthcare system requires systemic adaptation of the regulatory framework, updates to specialized training programs for pharmacy personnel, and consideration of local financial and economic indicators.
