Abstract

This chapter examines the optimization of pharmacotherapy for patients with coronary heart disease (CHD) and comorbid conditions through interprofessional collaboration between physicians and clinical pharmacists. Polypharmacy in this patient population increases the risk of adverse drug reactions, clinically significant drug interactions, and reduced treatment adherence. The study is based on a narrative literature review and the analysis of clinical cases using the HADS, MARS-5 scales, and the NHS England methodology. Particular attention is given to gender-related differences in drug metabolism (CYP450), psychoemotional status, and adherence to therapy. The findings highlight the important role of clinical pharmacists in identifying pharmacotherapy-related problems and improving the safety, effectiveness, and personalization of cardiovascular treatment.
