Abstract
The relevance of the study is обусловлена the growing need for scientifically grounded and effective approaches to trauma psychotherapy in the context of widespread and prolonged population traumatization caused by war. This situation increases the demand for methods that ensure not only effectiveness but also an adequate level of safety and tolerability. The article provides a theoretical, methodological, and empirically informed analysis of contemporary evidence-based approaches to trauma psychotherapy, with particular focus on the treatment of post-traumatic stress disorder (PTSD) across various therapeutic modalities. It is shown that despite strong empirical support, these approaches have limitations in cases of complex, chronic, or ongoing traumatization. The aim of the article is to provide a theoretical and empirical rationale for Deep Brain Reorienting as a neurobiologically oriented approach to working with psychological trauma, to analyze its applicability in the context of war-related traumatization, and to outline its place within the national scientific and practical discourse. The methodology is based on the analysis of contemporary neurobiological models of trauma, findings from randomized controlled trials, meta-analytical data on the effectiveness of selected psychotherapeutic approaches, as well as materials from clinical and supervisory practice and narrative reviews. Results. The place of neurobiologically oriented approaches, particularly Deep Brain Reorienting, is outlined, noting its limited representation in the Ukrainian scientific discourse. Its role within the system of evidence-based trauma psychotherapy is demonstrated, taking into account the limitations of traditional methods in working with chronic and repeated traumatization. It is shown that Deep Brain Reorienting provides access to early subcortical components of the traumatic response, allowing a reduction in affective activation without intensive exposure to traumatic content. This mechanism expands the possibilities of psychotherapy for individuals with heightened psychophysiological reactivity, dissociative manifestations, and limited affect tolerance. Empirical evidence indicates the effectiveness of the method in reducing PTSD symptoms, its high tolerability, and its potential for remote application.
