Abstract

In the methodological guidelines, pharmaceutical care for patients with ischemic heart disease and comorbid conditions is considered a multidisciplinary process within the “physician–patient–pharmacist” triad. The recommendations are intended to assist community pharmacists and clinical pharmacists in making effective decisions to improve the quality of pharmaceutical care for such patients. International strategies and concepts, as well as regulatory and legal acts of Ukraine governing the provision of pharmaceutical care, are presented, along with the principles of its organization at the national, sectoral, and regional levels. Emphasis is placed on the importance of evidence-based medicine and the role of effective communication. Practical aspects ensure the integrity and coherence of the scientific and methodological recommendations. The guidelines are intended for learners in medical and pharmaceutical education at all levels, participants in postgraduate education, pharmaceutical professionals, medical representatives, academic and teaching staff, and managers of healthcare institutions and pharmacy chains.
